Start-Of-School Forms

At the beginning of every school year, all guardians and parents are required to complete and submit several critical forms to safeguard the health and safety of their children. Most of these forms must be submitted every year to make sure critical student contact, medical and classroom information is up to date.

The easiest way to submit the forms is to follow the instructions on this page. If you'd like some more explanation about the process, please see last year's principal's letter on this topic-but return to this page to print and submit.

To print and submit the forms follow these steps:

Review the forms lower down on this page to decide what you need to submit (most parents should be able to use the "one-click" form packet described below).
Click the links on this page to print the needed forms.
Fill in and sign the forms.
Submit all the forms (except the CORI form) to the classroom teacher of your oldest child. Please place all the forms (except the CORI form) into a envelope with your children's names on it. You may provide the forms in person or send them in with your child.
If you are submitting a CORI form, you must present photo ID to school office staff at the time the form is submitted. Therefore, submit this form separately. You may do this at the school office during school hours.

Emergency Notification - One per family required every year
Phone Chain - One per family required every year
Weekly Dimissal Plan - One per family required every year (you may need additional if your children's schedule's vary)
Over-The-Counter Medication - Submit as needed or desired for your children.
Prescription Medication - Submit as needed for your children.
Do-Not-Photograph - Submit as desired for your children.
CORI Form - Must be submitted once every three years in order to volunteer at Nixon. New requirement: as of last year these must be submitted in person and with a picture ID.

Not included in the one-click packet above are additional forms that are not used by all parents, but should be considered. Please individually print any needed forms and submit them with the others.
