Form Overview and Instructions

Please allow up to six weeks for approval of your background check.

Anyone who would like to volunteer for any Nixon School activity (either at school or off site including any field trips, Odyssey, Science Fair, Monster Mash, Room Parent, Recess Monitor, working in the library, any PTO event, etc.) must submit a CORI Form for a background check by the Commonwealth of Massachusetts. It must be approved before you can perform any volunteer activities and the approval process may take up to six weeks.

It is a good idea for all parents (both Moms and Dads) and Guardians to submit a CORI Form so that the requirement will not keep you from volunteering at the last minute for any of your child’s activities.

If you submitted a CORI Form during the first year of the requirement, it is time to re-submit another form. If you are not sure if you submitted a form previously, submit one just to be sure. The Nixon School, in accordance with Commonwealth law, will request that each volunteer re-submit a CORI Form every three years to keep his or her approval current.

To download the form, please click here. After you have filled out the form, you must drop it off in person at The Nixon School main office. An authorized Nixon staff member must validate your ID when you make the submission.

A separate CORI Form is required for every school at which you plan to play any sort of volunteer role.

All information provided in the forms will be treated as strictly confidential and will only be reviewed by the Superintendent, principals and certain designees.

What is a CORI Form and why does it take six weeks to process?

The Commonwealth of Massachusetts’ “Act Further Protecting Children” (Chapter 385 of the Acts of 2002) took effect on February 25, 2003. It requires that all volunteers who may have direct and unmonitored contact with students submit to and pass a criminal background check. This includes any adult who plans to volunteer for any activity either at The Nixon School or with Nixon students involved in a school or PTO activity.

The background check will be conducted through the completion of a Criminal Offender Record Information (CORI) Form. After you submit the form, it will be forwarded to the Criminal Systems History Board (CHSB). Adults for whom the school has not yet received approval by the CHSB may not volunteer for any school activities. Therefore, it is essential that forms be filled out far enough in advance for the school to submit the form, for the CHSB to review the form, and then for the school to receive approval.

This entire process can take up to six weeks.
