Welcome to the new school year! In order to ensure your child’s safety and well-being at school, we need some critical information from you. You will be receiving one form on paper, and ask that you fill them out, and return them to school immediately. Copies are also available online.
- Student Biographical Verification Report-Please verify and return (only available at the front office)
- Emergency Notification Form-Please complete and return
- Illness Letter-Please save at home for reference.
- Over the counter Medication Form-Please complete and return
- Prescription Medication- Submit as needed for your child(ren)
- Interval Health History Form-Please complete and return
- Phone Chain Form-Please complete and return
- Do Not Photograph Form-Please complete and return
- Regular Weekly Dismissal Plan-Please complete and return
- NEW Volunteer Guidelines Form- OPTIONAL

- Criminal Offender Background Check (CORI) Form – Not included. For volunteers who do not have a current CORI in place. Available at the Front Office with valid ID 6 weeks prior to any volunteering in the school.
At this busy time of year, I know it can be challenging to remember to complete all of this paperwork, and the school has sometimes not had all information back until November. After trying several methods in the past, I have found a very effective way to help parents remember this important task. All of the children of the school will have been informed on Friday that if 95% of the forms are returned by September 10, they will all have an extra recess!
Some other important safety reminders:
The school will continue with the safety procedure of locking all outside entrances after the school day begins. In order to ensure that we know who is in the building at all times, we will continue asking all parents and visitors to enter through the main entrance, SPEAK TO SOMEONE in the office about who is expecting you, sign in at the office to obtain and wear an identification badge. With such a large staff, many of whom may not know every parent, we ask that even the most frequent volunteers follow this procedure.
We will also continue keeping the circle and drop of lane clear of all cars except handicapped vehicles at all times in order to ensure that emergency vehicles have quick access to the school building. We ask that you park in marked spaces even if you are just doing a quick stop to school.
We know that you want us to know where your child is at all times. As has been done in previous years, if your child is not here and we have not heard from you and cannot reach you or an emergency contact, we will call the police.
