Dear Parent/Guardian of Third Grade Student,

The statutes of the Commonwealth enable the Sudbury Public Schools to request that physical examinations be done at the Fourth Grade Level.  Physicals must be dated within 12 months of the start of fourth grade.  (For example, any physical dated after 9/07.)

This physical needs to be completed by your child's physician and returned to the Health Office by September 4th, 2008.  Most physicians prefer to use their own form, but if you would like a copy of the Massachusetts School Health Record please contact me in the health office.
